Requesting your immunization record
USIIS is a confidential, web-based information system that contains immunization histories for many Utah residents. Participation in the USIIS system is voluntary and not all healthcare providers may participate or submit vaccine records to USIIS.
To request your own immunization record, you must be of legal age (over the age of 18). If you are requesting the record of a minor child (under age 18), you must be the child’s parent or legal guardian at the time of the request.
You can request your immunization record in the following ways:
Option 1: Request a record from your healthcare provider
Option 2: Access records through the Docket web or mobile app
Option 3: Request a record from your local health department
All Utah local health departments use or are connected to Utah Statewide Immunization Information System (USIIS). Contact your local health department and ask them to print an Official USIIS Personal Utah Immunization Record for you.

Option 4: Submit a request to USIIS

To request your own or your child's immunization records, you can submit a support request. Make sure to fill out the form completely and attach a copy of your photo ID.
