About USIIS

The Utah Statewide Immunization Information System (USIIS) is a free, confidential, web-based information system that contains immunization histories for Utah residents of all ages. USIIS consolidates immunizations from multiple providers into one centralized record.

Only authorized users, such as health care providers, pharmacies, schools, licensed childcare programs, and public programs have access to USIIS. USIIS complies with HIPAA and state law to protect patient privacy. All providers and users must sign confidentiality agreements before they are given access to USIIS.

USIIS is populated with birth records of infants (through a link with electronic birth records) and immunization records from public and private healthcare providers

How to Participate

Read the USIIS Confidentiality andย Security Policy

Each user needing access to the system must complete and sign a School and Early Childhood User Confidentiality and Security Agreement.

Follow the instructions on the forms to submit your requests.

A Utah Department of Health staff member will contact you by phone or Email to confirm your request(s). Email USIIS Customer Support at usiistracking@utah.gov with any questions.

Healthcare Provider Facility

  1. Complete and submit the USIIS Provider Facility Enrollment Agreement.
  2. Read the USIIS Confidentiality and Security Policy.
  3. Each user needing access to the system must complete, sign, and submit a Healthcare Provider Facility User Confidentiality and Security Agreement.

EHR System Interface Implementation Process for Providers

Healthcare providers may submit immunization information to USIIS by two methods- by using the online web application and/or by submitting data from their electronic health record (EHR) system via data interfaces.
